PERSONALISED SUPPORTS DELIVERED BY A PASSIONATE TEAM

Disability Support Services Melbourne

According to the Australian Institute of Health and Welfare (AIHW), about eighteen per cent of Australian population (about 4.4 million people) live with a disability, with one in three (33%) requiring support services. In order to provide the best service to our participants, we stay up to date with current research and guidelines. We adapt our service to work with the needs of everyone in order to provide the most personalised services.

We provide supports all over Melbourne and assist participants with a variety of diagnoses. Our workers take a warm and compassionate approach to the support of our participants as opposed to a clinical approach.

OUR SUPPORT SERVICES

Personalised Supports Systems provide a range of supports including individualised support, group programs, and health and well-being. The scale of these services varies and solely depends on the participant’s requirement(s). Our individualised support program offers several services including personal care, social and community access, sleepovers and stays away from home, and technological and domestic support.

Individualised supports

Individualised supports

Personalised 1:1 supports where we match the most suitable worker for our participants and look to build upon a long lasting relationship.

Group Programs

Group Programs

Structured programs that cater for all ages, demographics and interests. Hosting some of the best facilitators in Australia we are sure to have you covered.

Allied Health

Allied Health

Tailored application of exercise prescription, nutritional advice and therapy assistance in regards to rehabilitation and strengthening, across all diagnosis.

HOW WE WORK

We make it our business to deliver support services in a fun and engaging way for our participants. We have built and continue to develop a team of youthful yet experienced support workers that can be seen as older siblings and role models, inviting their participants to relate to them with high levels of comfort, knowing they are safe to just be themselves.

We realise the importance of building an empathetic and long-lasting relationship between our support workers and participants. While some participants require intense rehabilitation support, others just want a friend who understands them whilst assisting them with personal care. As our history shows, we strive to build confidence in you and make you comfortable enough to consider us an extension of your family.

FAQs

The main tasks and duties of a disability support worker are to keep the participants they support safe and happy. To achieve this, every worker is trained to adapt their support to each individual. Supports can include community participation, life and independent skill development, personal care and more.

Disability support services are a collection of organised efforts that aid people with disabilities who require support to meet their various needs ranging from daily life assistance and skills to more complex support like equipment/home modifications.

Disability support services can be provided by a number of individuals including various therapists, medical experts, youth workers and support workers.

Disability support providers are organisations or individuals whose sole function is to organise and provide the best care and support for people with disabilities in a streamlined fashion. Personalised Support Systems operates in several domains spanning the disability support sector across Melbourne. We provide group programs and activities at our centre in Nunawading as well as in home support including sleepovers. As an organisation, we aim to provide services in a way that empowers participants to live their lives however they choose to.

My son Ben has a couple of independent support workers that he has formed relationships with over time, that he now engages regularly, but our recent introduction to Personalised Support Systems has been a real freedom shift in the way we source support. This is a TEAM approach. There is a team of enthusiastic committed energetic and empathetic workers, with your own dedicated team leader, who will strive to understand and meet your needs. Communication is near immediate, and response to requests has seen the benefits of flexibility and adaptation in motion with a team. Clients can meet and become familiar with different team members, establish common connections, and set foundations to build trusting relationships with those workers with whom they identify. We’ve found the flexibility suits our particular situation as Ben often makes last minute decisions or changes of mind: all our requests have been happily met, sometimes at short notice! We have also now established some regular shifts which are proving to be very successful, giving Ben a sense of direction and control over his life… and giving us, his parents, more freedom and control over ours. Ben loves having the company of people his own age, with similar interests and social attitudes…. he considers all the team now as his friends. Above all else we all feel relaxed and confident in the integrity and commitment of all the workers that are part of the PSS team and in the business model. It certainly represents greater freedom and less stress for our family.

PSS have been so amazing with my son who is on the spectrum amoung other things. He is complicated in a lot of ways and has had extreme behaviours, but the support workers that have worked with him have gone above and beyond to work on getting him more socialised, working on managing his anxiety, getting him out in the community and being a “big brother” type mentor for him while still listening to what I want for him long term as his mum. For a teenage boy, this was essential to him moving through very tough times and through all the Covid lockdowns etc. Their hub space is amazing and gives him a chance to socialise with other kids and have a fun time in a safe environment. PSS are very quick to respond to needs, work with goals and put forward new support workers (when need be) according to our requirements and preferences. A special shout out to say Daniel and George that have been working with jasper for a long time now have been so consistent with him and have done such an awesome job. So glad I have the support of PSS. Couldn’t recommend them highly enough :))

FERNANDO

Fernando

Director – The Yin

The disability sector has been Fernando’s main focus from the very start of his working life. Having undertaken studies in Exercise Physiology and worked with a wide range of clients for more than a decade, he founded Personalised Support Systems on a wealth of knowledge, bringing to the organisation a person-centred approach to care that is highly valued by all the families this business organisation serves.

Matthew

Matthew

Director – The Yang

Matthew redirected his career path to youth and disability supports after witnessing his mother’s joy from working in this sector. Bolstering his experience with studies in youth work, he established his own disability service before collaborating then officially joining forces with Fernando under the Personalised banner. Even while juggling operational responsibilities, Matthew loves to support participants and watch how they transform once a quality system is built around them.

George

George

Team Coach & Problem-Solver

Seeing the improvements good support workers made to his siblings with ASD, the disability sector was always a natural fit for George. After several years delivering disability supports inside and outside the family home, George now works as a team coach – inducting new members of the Personalised team and helping staff at all levels of experience to work through the challenges they encounter in the course of their work.

Peter

Peter

Music Phenomenon

From eagerly playing recorder and xylophone as a 6-year-old in the Netherlands, Peter’s musical prowess has, thankfully, expanded quite a bit over the years! Armed with youth work experience and a Social Science degree, he “accidentally” found himself in Australia, where he soon found a new passion working in the disability sector. Combining his two passions, Peter now develops modified group music programs aimed at people with a disability.

Laura

Laura

Art Guru

Laura has experienced both a home life with support workers coming and going, and the challenges and rewards of helping a loved one living with a disability achieve their goals. Combining this background with several years as a support worker and a bachelor’s degree in fine art, Laura heads up Personalised’s group art programs to share her dual passions by helping participants embrace their creative freedom and self-expression.

Jacinta

Jacinta

Performing Arts Star

Jacinta has vast experience in devising, implementing, and teaching successful performing arts programs, as well as an extensive background in performance across both stage and screen in Australia and overseas. Through the performing arts program she is heading up for Personalised, Jacinta looks to support her participants in developing their own creative voice, as they find expression, creativity, and connection in a safe and stimulating learning environment.

Holly

Holly

Zen Energiser Bunny

As a key support to Personalised’s management team, Holly is a mixture of naughty and nice: an amazing chef who specialises in treats that are healthy, a high-octane personal trainer who directs her energy into teaching low-impact Pilates, and a support worker whose bubbly personality is further buoyed by the joy she gets from helping to facilitate the growth and development of those who need it most.
