Support coordination is a service that helps people with disabilities or complex needs to access the supports and services they need. A support coordinator works with individuals to identify their goals and needs, and then helps them to develop a plan to achieve those goals. Support coordinators do not advocate for the individual, but rather they build the individual’s capacity to advocate for themselves. They empower individuals by providing them with the necessary skills, knowledge, and tools to express their needs and make informed decisions. This can include providing information about their rights and entitlements, assisting with communication and self-advocacy skills, and connecting them with advocacy services if needed.
